How Breath-Hold Radiotherapy Enhances Patient Comfort and Outcomes

In radiation therapy, precision is critically important. But achieving that precision in the presence of respiratory motion is not only about equipment or treatment planning. It’s also about the patient. For those who are able, breath-hold techniques guided by spirometry offer both clinical and personal advantages that make a measurable difference in care.

Breath-Hold or 4DCT? Why It Depends on the Patient

For patients with irregular breathing patterns or those physically unable to perform a breath-hold, 4DCT and Internal Target Volume (ITV) techniques are essential for managing respiratory motion. These protocols have a valuable place in clinical workflows.

However, close to 80% of patients are capable of performing spirometry-assisted breath-hold using the SDX System, and for those patients, a better option is readily available.

With SDX, these patients can benefit from:

  • Smaller treatment margins
  • Greater sparing of healthy tissue
  • Improved targeting precision
  • Smaller irradiated tissue volumes

In short, more precise treatments and a more manageable experience for patients.

A Patient’s Experience: Breath-Hold With Confidence

Charles, a liver cancer patient treated at Alta Bates, shared his experience using SDX during treatment. Despite living with both asthma and COPD, he quickly adapted:

“By the end of the first and second day, I was able to hold my breath quite a while… and I’ve got asthma and COPD, and I was able to do it nicely. It’s just maintaining your own positivity about the process. It might be a little strange at the beginning, but it all works out ultimately. Just relax and go with the flow, and you’ll have success.”

— Charles, SDX® Patient

His story reflects what clinical experience has shown time and time again: when patients are supported with the right tools and guidance, they not only adapt to SDX® but often find it easier and more empowering than they expected.

When Comfort Supports Better Care

In a clinical environment, comfort is often viewed as secondary or as a bonus, not a benchmark. But in breath-hold radiotherapy, patient comfort becomes a foundation for treatment precision.

With SDX®, breath-hold consistency is made achievable through clear visual feedback, real-time spirometry monitoring, and personalized coaching. And that changes everything, not just for clinical reproducibility, but for the patient’s own mindset.

“They really talked to me a lot and were reassuring the whole time. I think that’s key to going through something like this, too (feeling calm). And it really worked.”

— Charles, SDX® Patient

When patients are calm, informed, and confident, clinicians also benefit, with greater consistency between sessions, smoother workflows, and more reliable dose delivery. With SDX, empowering the patient means strengthening the entire care process.

There’s a Better Option for Most Patients

At DYN’R, we recognize the value of 4DCT/ITV for the 20% of patients who truly need it. But for the majority of breath-hold-capable patients, there’s an evidence-backed solution that offers more. And that is SDX®.

By enabling patients to perform consistent, comfortable breath-holds, SDX® helps clinics reduce margins, spare healthy tissue, and improve clinical outcomes while offering a treatment experience that respects the patient’s role in their own care.
